There are TWO ways to quote Group Health Insurance… The easy way is to ask a few basic questions about your business,  the age/gender of each employee, and do you know of any employee health conditions?  The result will be a prompt quote with great rates, but….  When you choose a plan and apply every enrolling employee will have to complete and sign a health history questionnaire. 

Now with your employees (and dependents) health information, such as height/weight, medical conditions past current and probable future claims plus prescription information they will run a new quote. This informed quote may easily be double the initial “blind” quote.

By submitting health information first we will get a more accurate quote!  Different companies will rate conditions differently which may result in large differences on quotes with medical information disclosed. 

 For an accurate quote please submit the  Employer Information form the Employee Census form and the Individual Employee Questionnaire or the online Online Individual/Employee Questionnaire.  

For your safety, and so employees may answer freely...   Please provide your employees the privacy of submitting the sheet in a sealed envelope or allow them to fax their individual information directly to us. They may also complete this form online. It’s probably  best if you don’t know their health conditions. In my opinion… you may be accused of discrimination if you had to terminate an employee and you knew their condition(s) was responsible for higher rates for the group.

 On receipt (e-mail, fax or mail) of the above Group Employer Information I’ll call to confirm information.  Then we will send your census information to multiple companies for quotes.  When we receive your quotes I’ll call again for an appointment to show you’re the quotes and go over your options.
